Is my camera feed uploaded anywhere?

No. All decoding happens locally in your browser. Your camera feed and uploaded images are never sent to any server.

Why did scanning fail?

Scanning can fail if the image is blurry, too dark, or cropped so the QR code is incomplete. It can also fail if the code is not a valid QR code. Try a clearer, well-lit, fully visible image.
